To acquire the Precision Spread Vanguard Medal, you must kill three foes in a row with headshots. You may accomplish this with any weapon you have, but the key is to make sure you defeat foes by repeatedly hitting them with headshots. The streak resets if you hit two and then hit something else in the chest or with ability, and you'll have to start the headshots all over again.
