You must put it in the middle of a group of enemies, preferably three, and then have it explode. To win the Vanguard Medal, the grenade must take out at least three adversaries when it goes off. The medal will not appear if you miss only two adversaries, and you will not be able to acquire it.

Waiting for an enemy spawn to occur during a Strike Playlist and then throwing it at the one in the center is one of the best methods to acquire the Demolition Expert Vanguard Medal. The enemies won't be able to respond until their disembark or arrival animation is complete, allowing your grenade plenty of time to charge up. Make sure the Demolition Expert Vanguard Medal icon appears at the top of your screen when you hurl the grenade and it goes off. This means you've earned the prize and completed the challenge.
