The 14th mission in the Call of Duty: Modern Warfare 2 campaign is Prison Break. You play as John "Soap" McTavish and are joined by Rodolfo "Rudy" Parra and Simon "Ghost" Riley. You have been sent on a mission to free Alejandro and his men from the prison Graves and Shadow Company have imprisoned them in for unknown reasons.
When you locate Alejandro's prison cell, you could be considering how to set him free. The door is inaccessible; you cannot open it, smash it, or detonate it. There aren't any remote controls nearby or anywhere else in the space. The problem is that Ghost would be waiting at the entrance with a pair of bolt cutters if the game were functioning properly. All he needs is for you to push the door open. If Ghost is nowhere to be found, you've probably encountered a glitch where Ghost gets stuck in the mission's earlier phase. The good news is that there is a workaround that doesn't involve starting the level over completely.
How to fix the Prison Break glitch and free Alejandro
Look for Ghost by going back the way you came through the prison block. He may be difficult to see because he is glitching. Make sure he hasn't become stuck in any walls, doors, or containers by carefully inspecting any dark corners and tight spaces. Keep searching. He must be present someplace.
Throwing a flash grenade at him when you find him seems to be the best way to release him from glitchy limbo. He should then be able to stagger free, after which he will rush over to Alejandro's cell and retrieve the aforementioned bolt cutters. Alejandro will then be released once you interact with the door.
